Use for one-time partial withdrawals, systematic withdrawals, Required Minimum Distributions, Cash Value surrenders, Roth IRA conversions, and electronic deposit set up.

Non-Financial Change Form

Use for address changes, beneficiary changes, name changes, Ownership changes, and PIN changes.

Request for Scheduled Annuity Payments

Use to request scheduled or early annuitization - also use for annuitization under a death claim option.

Lost Policy Affidavit

Use to request a duplicate policy or to surrender a policy (signed surrender form also required.)

Annuity Death Claim Form

Use to file a death claim for all IRA and non-IRA annuities, with the exception of SPIA contracts.

Use for Trust-owned cases or Trust beneficiaries (to confirm Trustees, signing authority, etc.)
